This notice describes how medical information about you may be used and disclosed, as well as our policies regarding appointments and medication prescriptions. Please review it carefully.

We may disclose or share information about you with CHS staff in order to coordinate the care you receive. CHS staff includes, psychiatric providers, therapists, psychologists, nurses, case managers, liaison case managers, community support staff, administrative personnel, billing, and support personnel.
Cancelling or Rescheduling an Appointment:
We ask that you notify our office as soon as possible if you must reschedule or cancel your appointment. Any appointments cancelled or rescheduled with less than 24 hours’ notice will be considered a no-show. Three consecutive cancellations will be reviewed by the provider for determination of continued services.
A “no-show” will be documented when you fail to be present at the time of a scheduled appointment or when you cancel a scheduled appointment with less than 24 hours’ notice. Any established patient who fails to show or cancels/reschedules an appointment and has not contacted our office with at least 24 hours notice will be considered a no show and could result in a charge of $60.00.
Therapy Services & Medical Management Services:
Showing up over 15 minutes late for a scheduled appointment may result in the provider to no longer see you that same day and may be considered a “no show”. If three "no-shows" are documented within a six-month time period, no further appointments will be scheduled. You may appeal this decision.
Medication Refill Policy:
Refill request require up to 72 hours of notification prior to determination of approval or denial. If you would like to request a refill of a current medication, please contact your pharmacy and ask a faxed request be sent to the CHS office. At the discretion of the medication provider, refills on medications may be approved until the time of your next scheduled appointment. Prescriptions will not be refilled until an appointment is scheduled.
